Attention:

Requirements:

For registrants who belong to the Sacred Heart Parish-Shrine: 

1. Have a photocopy of your children’s BIRTH CERTIFICATE with the Registry Number.
2. Make sure the children to be baptized must be NOT MORE THAN SEVEN (7) YEARS              OLD.
3. Children aged 7 years old and above need to be interviewed by a priest and if need be, must undergo catechism. This process will take about a few days.
4. Fill out the registration form at the parish office.

For registrants who DO NOT belong to the Sacred Heart Parish-Shrine (outside Barangays Kamuning, Sacred Heart, Obrero, Kristong-Hari, and South Triangle)

1.  First, get an official letter of a permit or Permit to Baptize from the parish where you belong.
2. Have a xerox/photocopy of your children’s BIRTH CERTIFICATE with the Registry Number.
3.  Make sure the children to be baptized must be NOT MORE THAN SEVEN (7) YEARS OLD.
4.  Children aged 7 years old and above need to be interviewed by a priest and if need be, must undergo catechism. This process will take about a few days.)
