Sacrament of Baptism - Updated Schedule

Attention:

1. Due to the pandemic, there are changes in the schedule of the celebration of the Sacrament of Baptism.
2. The baptism schedule is available daily by appointment. On Sundays, the schedule is at 1:00 p.m.

Requirements:

For registrants who belong to the Sacred Heart Parish-Shrine: 

1. Have a photocopy of your children’s BIRTH CERTIFICATE with the Registry Number.
2. Make sure the children to be baptized must be NOT MORE THAN SEVEN (7) YEARS              OLD.
3. Children aged 7 years old and above need to be interviewed by a priest and if need be, must undergo catechism. This process will take about a few days.
4. Any of the child’s parents can fill out the following Registration Form submit it to the office together with the Birth Certificate of their children. Click here to download the form.

For registrants who DO NOT belong to the Sacred Heart Parish-Shrine: 

1.  First, get an official letter of a permit or Permit to Baptize from your own Parish Priest.
2. Have a xerox/photocopy of your children’s BIRTH CERTIFICATE with the Registry Number.
3.  Make sure the child/ren to be baptized must be NOT MORE THAN SEVEN (7) YEARS OLD.
4.  Children aged 7 years old and above need to be interviewed by a priest and if need be, must undergo catechism. This process will take about a few days.)
5. Any of the child’s parents can fill out the following Registration Form and submit it to the office together with the Birth Certificate of their children and the Permit from their Parish Priest. (Online submission accepted). Click here to download the form.
